Athletic Clearance Process
1. Complete the Athletic Registration form.
2. Submit a Pre-Participation Physical Form (or Health Update Form*) and complete Concussion/ImPact Testing with the Athletic Training staff.
Please bring your fully-charged Chromebook when you hand in your paperwork. Please check your sport's Google Classroom for collection dates and times.

*Physicals are valid for 365 days. Submit a Health Update form if you currently have a valid physical on file with the Athletic Training staff. Click here to check your physical date/clearance status.
3. Receive clearance from the school physician (this may take 1-2 business days).
4. Receive academic eligibility clearance from the Athletic's Office.
